Sulpiride

 

 

INDICATION & DOSAGE

Indications:

Dogmatil 50: Neurotic state with inhibition; psychosomatic component of organic diseases.

Dogmatil 200: Acute psychoses; chronic psychoses (schizophrenia, non-schizophrenic chronic delusional states: Paranoid states, chronic hallucinatory psychoses).

 

Dosage:

Dogmatil 50: Adults: The daily dose is 2-3 caps/day for a maximum period of 4 weeks.

Dogmatil 200: Adults: The daily dose is 200-1000 mg.

 

Administration:

May be taken with or without food.

 

 

CONTRAINDICATIONS

- hypersensitivity to sulpiride or to one of the other ingredients of Dogmatil

- prolactin-dependent tumours (eg, pituitary gland prolactinomas and breast cancer)

- known or suspected pheochromocytoma

- in combination with sultopride, dopamine agonists (amantadine, apomorphine, bromocriptine, cabergoline, entacapone, lisuride, pergolide, piribedil, pramipexole, quinagolide, ropinirole) apart from in patients with Parkinson' s disease

- breastfeeding

- in combination with alcohol and levodopa

- medicinal products liable to induce torsade de pointes

- certain neuroleptics (thioridazine, chlorpromazine, levomepromazine, trifluoperazine, cyamemazine, amisulpride, tiapride, pimozide, haloperidol, droperidol), and other medicinal products eg, bepridil, cisapride, diphemanil, erythromycin IV, mizolastine, vincamine IV, halofantrine, pentamidine, sparfloxacin, moxifloxacin, etc

- dopamine agonists (amantadine, apomorphine, bromocriptine, cabergoline, entacapone, lisuride, pergolide, piribedil, pramipexole, quinagolide, ropinirole) in patients with Parkinson' s disease

 

Dogmatil 50: Due to the presence of lactose, Dogmatil 50 is contraindicated in the event of congenital galactosaemia, glucose/galactose malabsorption syndrome or lactase deficiency

 

 

PRECAUTIONS

- Neuroleptic Malignant Syndrome

- signs of vegetative dysfunction

- patients requiring treatment with a neuroleptic

 

Special Precautions for Use: Dogmatil 50/200 must not be used in the event of Parkinson' s disease, apart from in exceptional situations.

 

- in the event of renal impairment, reduce the dosage and increase patient monitoring

- epileptic patients

- alcohol consumption and use of medicinal products containing alcohol

- drivers and machine operators, in particular, must be made aware of the risk of drowsiness associated with the use of Dogmatil 50/200.

- lactation

- children

- elderly

 

ADVERSE DRUGS REACTIONS

 Neuropsychiatric Disorders:

-early dyskinesia

-extrapyramidal syndrome

-hyperkinetic-hypertonic

-excitomotor; akathisia

-tardive dyskinesia

-sedation or drowsiness.

Endocrine and Metabolic Disorders:

-transient hyperprolactinaemia

-reversible on treatment discontinuation, which may induce amenorrhoea, galactorrhoea, gynecomastia, impotence or frigidity

 -weight gain

 Cardiac Disorders:

-torsade de pointes .

 Neurovegetative Disorders:

 -Orthostatic hypotension

Neuroleptic malignant syndrome

 

 

INTERACTIONS

 

-Sultopride

 Increased risk of ventricular rhythm disorders, particularly torsade de pointes.

-Alcohol

 Alcohol increases the sedative effect of neuroleptics.

 Driving and using machines may be dangerous due to impaired vigilance.

 Avoid consumption of alcoholic beverages and use of medicinal products containing alcohol.

-Levodopa

 Mutual antagonism of levodopa and neuroleptics.

 In patients with Parkinson' s disease, use the minimum effective dose for both medicinal products.

-Dopamine agonists (amantadine, apomorphine, bromocriptine, cabergoline, entacapone, lisuride, pergolide, piribedil, pramipexole, quinagolide, ropinirole) in patients with Parkinson' s disease.

-Halofantrine, Pentamidine, Sparfloxacin, Moxifloxacin

 Increased risk of ventricular rhythm disorders, particularly torsade de pointes.

-Hypokalaemic agents (hypokalaemic diuretics, stimulant laxatives, amphotericin B [IV route], glucocorticoids, tetracosactide): Increased risk of ventricular rhythm disorders, particularly torsade de pointes.

-Combinations to be Taken onto Consideration: Antihypertensive agents (all types) exacerbated risk of orthostatic hypotension and antihypertensive effect (additive effect).
